believe me,that this icon file contains all the sizes mentioned,you see only 128x128 probably because all your icons are in that size-your system is set up on that size.But try it open this icon file in Icon Packager or IconWorkshop, Icon Developer and you'll see all the sizes included(8 sizes in total).This icon has been packed as a single ".ico" file which contains 8 sizes(they are not packed individually).
