Actually that is the default skin of Windows Media Player 11 Beta for Windows XP (SP2).
The official link to download it is: Link.
It is great specially visually speaking. Performance is also great but with some problems specially in the library section (of course since it's beta...).
If you do decide to download it, here is a little "trick" to make it load a certain playlist when you run it. (A major flaw in this release is that whenever you close it and open it again the playlist is always empty instead of keeping the last used playlist):
You can change the shortcut for WMP to something like this:
"C:\Program Files\Windows Media Player\wmplayer.exe" /Playlist "playlist name"
Of course, you need to put your actual path for wmplayer.exe, and your playlist name. All the " are needed.
Btw, that way it will open in "Now Playing" tab, if you prefer it to open at "Library", you could add: /Task medialibrary

Link
Just click on the "Download to Desktop" button under the preview image.
It's a single rar file containing both themes plus a program to remove the "Log Off" and "Turn Off Computer" text on the Start Menu provided you are using an English version of Windows XP Service Pack 2, though I haven't tested that program yet..
Both themes (and program) are from jemaho.
PD: Both themes are newer versions than the ones I used on the screenshot, believe me they are even better than the ones I used
