Just Jane! this is not a WindowBlinds skin. For WB use this - http://filkose.wincustomize.com/viewskin.aspx?SID=5316&skinid=4791&libid=1&UID=423998. I know it's confusing because the icon is the same as for WBs. 
Hopefully, it will be distinguished in the future.
As far as this download, it's a part of a skin -- Toolbar Icons. This is installed just by clicking on the download button. Then you need to open and configure WindowBlinds; on the left side click on My Toolbar, and then select second or the third option (whichever suits you the best), and Viola! Your new toolbar is ready to go...
